Make a Referral

Every referral is thoroughly assessed by our Senior Management team. We can provide an initial assessment inside a week if required.

The Process

Our referral and assessment process includes:

  • Sourcing a comprehensive set of notes for every person, covering their psychiatric, forensic (if applicable), risk and personal history. We also gather any available specialist reports in areas such as psychology, occupational therapy and nursing
  • Two of our senior management team will then make an initial assessment, virtual or face-to-face in their environment, based on these notes
  • We have a discussion with the ward manager and/or the named nurse if they are being discharged from hospital
  • We also have a discussion with their social worker and/or care coordinator
  • Where it's appropriate, we also have a discussion with their family members
  • If the referral is for residential care or supported living, we make arrangements for them to visit our site. While they are with us we carry out a second face-to-face assessment and observe how they interact with staff and existing residents
  • If needed, we have an additional face-to-face assessment and/or request for further information

At the end of the assessment process, we create a detailed assessment report, which includes our decision, proposed support plan, risk assessment and costs.
