Community Outreach

Our community outreach service is designed for adults living in their own homes, or with their families, who need support to live more independently. Our specialist outreach team deliver person centred support to help maintain and develop independence.

Our services are designed to meet the specific needs of the individual we support and cover a wide range of personal and social care needs.

For example, we can provide support in assisting with cooking nutritious meals, cleaning, and assistance with learning how to budget. We can support with aspects of personal care and support in taking medication. Supporting to become involved in community activities, such as attending college, employment, volunteering, or enjoying hobbies and taking part in sports.

Our skilled outreach team will also encourage service users to develop their own skills and strengths and how they could use these to contribute to their local community. They will help develop existing skills or to learn new ones that will assist engagement with other people and to develop a greater sense of 'citizenship'.

We provide daily support - up to seven days a week. Flexible support is available for a minimum of two hours, up to 24 hours a day.

Community outreach is another step on the care journey to the destination of full independence. But whether a stop on the road or a destination, our goal is to provide the perfect blend of support and self-reliance needed by every person we support.
